Have you considered Supple Nipps? These are suction cups made primarily for women who have "innie" nipples who want to breastfeed, but they work great for men. They were quite small before I started using Supple Nipps.

What size do I should start with?

I started with the #2 regular size, (not the 2L, which is half an inch in diameter. I just barely fill it, although when I first got them I filled only half of its length. After that you can graduate to the 2L for longer nipples, or if you want a wider diameter move on to the 3 or 4.
