How about trying a bra "extender". These are short strips of bra band that have multiple hook positions on the length. You clip on the extender to each end of the bra band and presto! You have a bra of the correct cup and band size and one that you could never buy anywhere. Check Amazon for "bra extender", or make your own.
For standard bras the above works well, but I had a difficult case to share. I recently purchased some Junior Misses bralettes to get the correct cup size in a sheer lace (wanted something sexy for my gurls) but the biggest band available was way too small. I did look at extenders but none matched the style or width of the bra band. I also looked at making my own extender which is easy to do with some ribbon or elastic. The lace was very difficult to match so I just ordered a duplicate bralette and will cut up the band to make an extender that is a perfect match! Of course these bralettes are very inexpensive so it is the best choice of fix for me.
I was also thinking of some fancy solutions like cutting the bra band in the front and replacing the band between the cups with some ribbon or chain or something blingy. Bra jewelry! By changing the front and extending the back then you can place the cups where you really need them. I have "outward apex" so I need to bring the cups closer together to make me happy and cutting the band in front is the only way to adjust everything.
Just some ideas on what to do when you are too tall or too whatever for main-stream sizing.
