Can only give you my thoughts on Q2.... I to had similar problems so now wear invisible bralette tops, they are seam-free, have wide shoulder straps, thin padding and are a really thin and stretchy material.
As such they easily hide under even the tightest of t-shirt, holds everything in place and stops nipples showing through.
It took me a bit of trial and error to find the right ones, so best way is to look for the ones with smooth / seamless edges, with no visible edge seam.
Also, be wary of the colour, I presumed white was the best for not showing through, but now realise that nude/camel colours are best, especially if wearing a white T / top.
