I really do understand the frustration, at 50 though, a few extra weeks or months for your doctor to be prepared is totally reasonable imo. Honestly, you may just want to have him send a consult to a doctor who has dealt with it before. Especially if he is going to be the one prescribing it.
I do understand although i have not gone to the extent you have with doctors, I have had the same feelings.
