Is your doctor TG friendly? That makes a big difference. I'm scheduled to see my new doc for my 6 mo. check up and HRT scrips next month. When I called for an appointment they asked what name and pronoun I wanted to be called by, so I said "Clara" and "she/her". I'll show up dressed in femme clothes (jeans, top, underwear), wig, and a little makeup.
Now if I was going to my old doctor who always knew me as male, yes, I would be nervous about what to say. I don't know why it should matter, but it does.
