For me, I lost breast volume on 50mc patches. It took my Dr 6 monthe to finally prescribe me 2x100 patchs ( twice weekly ), before I started to gain volume again.
The best estrogen readings I gat as prescribed was @220pmol ( just above menopausal level ) if i left them on as I cycled new patches I managed 265pmol.
My Drs answer .. More spiro.
With injections, my last bloodwork was @ 1300pmol,
and my breasts still do not look as good as yours.

I have enough with low dose it seams.
High dose only gave me more side effects without more effects.
I feel calm and feminize better on lower dosages. Strange how human biology can be so different for every person.
We live in the Westearn world where doctors try one approach for everyone. A doctor who tries to think along with you instead of dictating you what to take, is so much better.
I tried Prometrium, progesterone, and I hated how it made me feel.
I tried Spiro and Androcur, both caused depression and excessive prolactin ( at risk for prolactinoma ).
Now on just a lower dose of Estradiol in transdermal form, no anti-androgens apart from Spearmint and Peppermint which I recently stopped taking because I hated chewing that down every day lol, and I feel awesome. I leave the 4 older patches on while applying a fifth new one. I expect my Estradiol levels to be around 700 pmol/l now since with 3 patches ( 2 old and 1 new ) they where around 500 pmol/l.
My E drives that nasty T down by itself. Muhahaha :-P
