You probably could get a total T test authorized by complaining to your doctor about feeling a loss of energy and a waning sex drive, and asking if it might be a case of low-T. With all the advertising for testosterone boosting drugs (e.g., Androgel) on TV these days, doctors must be seeing more and more men patients asking for a serum testosterone test.
Of course, if you've been on PM for several months like I have, your T is probably going to come in on the low, low side. That will prompt your doctor to prescribe a drug to bring it back up. If you refuse the drug and are asked why, what are you going to say?
